POLICY & PROCEDURES

ATTENDANCE: An important part of learning to dance is attendance. It takes time and perseverance to
build the strength and learn all the techniques involved in dance. Dance looks easy, but dance lessons
are far from easy. Be patient and supportive of your child. We urge parents to enroll their child with the
intention of completing eleven months. When a student misses class, it not only affects him/her, but it
also affects everyone else in the class. If your child must miss class please call and notify the office.
Students must not go outside for any reason until their ride arrives. Students under 7-yrs of age must
have their parents come into the building to pick them up. Please be on time for drop off or pick-ups.

MAKE UP CLASSES: Make up classes may be scheduled at a time that is convenient for both you and
the studio. There will be no refunds for missed classes. In the event of excessive absenteeism, the
instructor has the right to ask the student to take a lower level class and/or dismiss that student from the
showpiece, even if the costume has been paid for with no refunds. Students that quit and start back in
the same year will be asked to repay a registration fee.

ATTIRE: Proper attire (including shoes) is necessary to participate in class. Those that do not have
proper attire must sit and observe class. Parents will be notified. This is for the safety of your child. (No
street shoes or jeans.)

TUITION & REFUNDS: There will be no refunds for any reasons. If you decide to discontinue a class,
you will provide a written notification two-weeks prior to the next month. You will be billed continually
until you let us know. If monthly payments are not being made, late fees will be added. Monthly rates
stay the same, even though class days vary per month. Refer to website for Summer class rates. Tuition
is due by the 5th of each month. If your tuition is received on or after the 6th of the month a $5.00 late fee
will be charged. There is a $30.00 fee charged for all returned checks.

Tuition may be mailed to Chris' Upstage Performance Studio, P O Box 1461, Mead WA 99021, or
dropped in the tuition box inside the studio. We will not send you a bill unless your payment is late.

MEMBERSHIPS: Platinum, Gold and Silver Memberships require a 12-month agreement. If you cancel
your membership prior to the 12-month term of your agreement, you agree to be held responsible for the
full balance of the membership plus a 10% cancellation fee. A 30-day cancellation period is provided
without penalty from the first day of acceptance.

PAYMENTS: You are encouraged to use our automatic payment program provided for you via
http://upstageperformancestudio.com/shopupstage.html. If you do not use our automatic payment
program and your payment is not received on or before the 5th of the month tuition is due, a 10% late fee
($5 minimum) will be applied to outstanding balances. If, within that same month, your payment is not
received by the last day of the month tuition is due, an additional 10% late fee ($5 minimum) will be
applied every month until payment is received. If payment is not received within 60 days, from the 5th of
the month tuition is due, your account is in default.

DEFAULTS: If your tuition or membership is 60 days overdue you agree to pay any and all collection, court
and attorney fees provided by law.

CLASS PLACEMENT: Class placement is determined at the time of registration, based on the student's
age and experience. Our staff will evaluate students during the first week of class to assure proper
placement when there is any doubt about a students level. We place in the lower level so those students
may be advanced rather than moved back if adjustment is required. Please be patient with schedule
changes, which may occur to accommodate all students.

CONCERNS: Chris' Upstage Performance Studio will try and please all our student and parents
needs. Please do not interrupt the staff during class times, with questions or complaints. Call and leave a
message for the teacher you need to speak with, or speak to the front desk staff or leave a note in the
tuition box. Staff phone numbers will not be given out. Instead we will provide your phone number to the
teacher you need to speak with. If the problem is not remedied after talking with the teacher, a call or
appointment can be made with Chris' Upstage Performance Studio.

CHOREOGRAPHY: I will ask permission to use any of Chris' Upstage Performance Studio
choreography and give credit to Chris' Upstage Performance Studio and appropriate choreographer,
if allowed to use choreography.

PHOTOS: All photos and videos taken by the studio or by a professional of my child or myself may be
used in studio promotions.

END OF YEAR RECITAL: Our year-end show is a great experience, but it is optional. We hope everyone
plans to participate. The $30.00 recital fee is a non-refundable fee, which goes towards the cost of theatre
rental, tech run-through fees rehearsal time, insurance, security, programs, stage surfaces, props sound
and lighting. The fee does not cover tickets for your family, guests and friends. Our show normally falls
during a weekend, at the end of June. Not all classes on the schedule participate; please ask staff for
more details. Costume fees for each class range from $0 -$100. You may be asked to buy special shoes,
tights, make-up, etc. Costume fees are due in full by December. Those interested in paying toward their
costume may do so on a paying plan with a minimum deposit of $25.00. Costumes may need altering. It
is your responsibility to alter your own costumes.

GENERAL RULES: No smoking in the studio. No gum, food or drink in the dance rooms or voice rooms.
Only dancers enrolled in each specific class are allowed in the dance rooms or voice rooms. Homework
is
to be done before class or after class, not during class. Chris' Upstage Performance Studio is not
responsible for injuries sustained at the studio or in conjunction with studio related activity. Do not leave
siblings unattended in the studio. Chris' Upstage Performance Studio is not responsible for lost or
stolen items. No mobile phones or pagers to be brought into the dance rooms. The telephone is for
studio personnel only. Policy & Procedures and Release from Liability, Waiver of Claims & Assumption of
Risk must be signed to take class. No exceptions.

By accepting this Policy & Procedures, I affirm that I have read the Policy & Procedures in full, that I
understand its terms, and that I agree with those terms.

* Changes to this agreement may occur without notice. Therefore, you are encouraged to print a copy of
this agreement at the time of your registration.
